*If your student needs any type of medication at school, and it will be administered by School Staff, you need to take a Form 1 to a medical professional before we can administer any medication at all. This includes EpiPens, inhalers, antibiotics, and allergy medication . Bring your completed Form 1 to the office to meet with the VP. You will also need a copy of the Pharmacist's instructions for staff

Bring your completed Form 2 to the office to meet with the VP. You will need a copy of the Pharmacist's instructions for staff.
